Nurse (Clinical Care Coordinator)

Coordinates appointments and procedures for patients who need further care or continuity of care at other facilities. Develops and maintains iCare patient panels for patients with chronic disease. Coordinates with Outpatient DME coordinator to obtain equipment and needed supplies for patients, works with allied medical services or outside vendors as needed. Assists with arranging appointments for patients within the IHS and at contract facilities. Performs comprehensive professional nursing duties for the care of patients with a variety of injuries and illnesses in the outpatient department. Performs a wide range of nursing care for patients in all age groups and their families who present an unlimited range of different health related problems. To qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; social). Education:
A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. OR Degree from Foreign Nursing School:
Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools. Licensure:
For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. Management of nursing care in outpatient primary care environments and emphasizing interdisciplinary collaborative processes to assess, advocate, plan, implement, coordinate, monitor and evaluate health care options and services which are consistent with patient-centered medical home principles. Experience in coordination of patient care and resources for a caseload of patients across the continuum of care and through care transitions; able to provide patient and family education pertinent to a wide range of conditions and health factors; skilled in supporting patient self-management. The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
